IN-PROCESSING:

Every client is asked to complete in-processing forms. The information requested serves more than one purpose. The medical/health related information will be used to ensure your safety. There are certain medical conditions that may limit the areas treated, or may even prohibit treatment altogether. The information requested will not be shared with anyone outside of Sunflower Massage, our insurance biller, and your insurance company without your permission.

BOOKING ON-LINE:

Please be aware that failure to keep an appointment without a 4 hour minimum advance notice may result in a No-Show fee. The is an unfortunate necessity.

BILLING:

General - Payment is expected when services are rendered.

Credit Cards - Credit cards accepted include VISA, MasterCard, Discover, and American Express.

Insurance - Those receiving medical massage therapy covered by insurance are asked to pay only the amount not paid by their insurance.

Cash - Cash is accepted. However, our ability to make change may be limited.
